  • We provide services to clients ages 5-26 years old. 
  • We contract with most insurance companies, including HFS Medicaid and MCO Medicaid companies. 
  • Please call (217) 531-2360 or email hopesprings@cunninghamhome.org  with questions about insurance coverage.

Get started

  1. Complete our HIPAA-protected online client referral form below or call our office at (217) 531-2360.
  2. Once referral information is obtained, a HopeSprings representative will contact you within 24 hours to discuss our services and insurance processes and will schedule an intake appointment with a therapist.
  3. For referrals from DCFS or POS agencies, print and complete this intake paperwork.
  4. If desired, you can view and print our client handbook. A client handbook in Spanish is also available.
