  • We provide services to clients ages 5-26 years old. 
  • We contract with most insurance companies, including HFS Medicaid and MCO Medicaid companies.

Get started

  1. Complete our HIPAA-protected online provider referral form below or call our office at (217) 531-2360.
  2. Once referral form is received, a HopeSprings representative will contact you and/or the client within 24 hours.
  3. DCFS and POS referring partners: complete the online provider referral OR fax a completed DCFS referral form to (217) 352-2635.
  4. If desired, you can view and print our client handbook and fee agreement for information purposes.
